This innovative program fills a gap in the continuum of health services available to Nova Scotians between hospital care and community. <\/p>\n\n\n\n<p>At the Transitional Care Centre, patients will partner in their care planning and work with clinicians, like occupational therapists and physiotherapists to prepare for their return home. Care and support will vary depending on individual care plans.The Transitional Care Centre will provide services to clients in the Halifax Regional Municipality and West Hants region of Nova
