Transitional Care Centre

Shannex will operate a new Transitional Care Centre located in West Bedford, Nova Scotia, opening late 2024.

The Transitional Care Centre will serve patients of Nova Scotia Health’s Central Zone (18 years and older) who no longer require a hospital stay, but need additional support before they return home.

This innovative model of care fills a need in the continuum of health services available to Nova Scotians between hospital care and community. Adding this new service is expected to reduce emergency wait times and capacity challenges faced by Nova Scotia’s health care system by freeing up hospital beds. Patients will be partners in their care planning and will collaborate with clinicians, like occupational therapists, physiotherapists, and others to prepare for their return back to home. Care and support will vary depending on individual care plans.

What is transitional care?

Transitional care can apply to any situation when someone moves from one care setting to another. In this case, it’s a level of care when a person is discharged from acute care but requires additional support before returning to home (or a new home). Supports include helping patients and families with medical, rehabilitative, practical, and emotional needs in a community care setting.

What will be different about the Transitional Care Centre?

Currently, Shannex communities have an accommodations-based relationship with residents who call our locations home. In the Transitional Centre, we will serve patients of Nova Scotia Health who do not live with us – they will be staying with us for a shorter duration as is required to get them on the path back to home/community.

Where will the patients come from?

Patients will be transferred from one of Nova Scotia Health’s acute care environments (e.g., hospital) after being referred directly to the Transitional Care Centre. Patients may also be directly admitted from a home environment. This is not a walk-in community.

How long will patients stay at the Transitional Care Centre?

A patient’s stay will depend on their personalized care plan. No matter how long a patient stays with us, their experience will be transformative and focused on a temporary treatment plan that enables them to return to a place they call home – this can include their own home or with loved ones, a community-style environment (e.g., retirement living community) or a long-term care environment.
