We are experts in community care

Every day, we see the difference personalized and appropriate care planning, delivered by skilled team members, makes to our home care, long term care and retirement living clients.

Transitional Care Centre

Shannex will provide health care services at Nova Scotia Health’s new Transitional Care Centre located in West Bedford, Nova Scotia.

The model of care offered at this centre is designed to provide enhanced short-term services for individuals who have completed their acute hospital stay but require additional support before returning to the community. This innovative program fills a gap in the continuum of health services available to Nova Scotians between hospital care and community. At the Transitional Care Centre, patients will be partners in their care planning and will work with clinicians, like occupational therapists, physiotherapist and others to prepare for their return back to home. Care and support will vary depending on individual care plans.

The Transitional Care Centre will provide services to clients in Central Zone (Halifax Regional Municipality and West Hants).

Opening 2024

What is transitional care?

Transitional care can apply to any situation when someone moves from one care setting to another. In this case, it’s a level of care when a person is discharged from acute care but requires additional support before returning to home (or a new home). Supports include helping patients and families with medical, rehabilitative, practical, and emotional needs in a community care setting.

Meet Vanessa Quigley,
Director of Restorative Care

“As a registered nurse, I’ve spent years working in acute care, emergency care and the flow of patients in and out of hospitals in Nova Scotia. At the Transitional Care Centre, Shannex will provide patients with the personalized care plans they need to return to wellness and return home – whatever home looks like for them.

The Transitional Care Centre is the first of its kind in Nova Scotia – a new model of care and service that encourages health innovation and collaboration. It’s really exciting to be part of, and I look forward to welcoming our first patients.”
