West Bedford Transitional Health
With phased opening beginning in December 2024, West Bedford Transitional Health is operated by Shannex with services provided by our skilled team of healthcare professionals. It is located on Hogan Court in Halifax, NS, accessible via Highway 102 and the Larry Uteck exit.
West Bedford Transitional Health will serve patients of Nova Scotia Health’s Central Zone (18 years and older) who no longer require a hospital stay, but need additional support before they return home.
This innovative model of care fills a need in the continuum of health services available to Nova Scotians between hospital care and community. Adding this new service is expected to reduce emergency wait times and capacity challenges faced by Nova Scotia’s health care system by freeing up hospital beds. Patients will be partners in their care planning and will collaborate with clinicians, like occupational therapists, physiotherapists, and others to prepare for their return back to home. Care and support will vary depending on individual care plans.

We are experts in community care
Every day, skilled team members partner with patients to provide personalized and appropriate care planning. Key roles include:
TCA (CCA)
Certified Continuing Care Assistants (CCAs), referred to as Transitional Care Assistants (TCAs) provide personal care with a focus on supporting and encouraging the level of functional ability required to return to community.
Allied Health
Restorative health professionals (e.g. PT/OT and PT/OT Assistants) support the focus on functional abilities, encouraging adherence to treatment plans, and guiding patients with their expertise and knowledge
Nursing Staff
Licensed Nurses (LPN/RN) are integral in supporting the goal of returning home by promoting health, being engaged in inter-professional collaboration, performing regular assessments, and offering evidence based clinical expertise to inform care plans that fit with patient/family goals.
What is transitional health?
Transitional health can apply to any situation when someone moves from one care setting to another. In this case, it’s a level of care when a person is discharged from acute care but requires additional support before returning to home (or a new home). Supports include helping patients and families with medical, rehabilitative, practical, and emotional needs in a community care setting.

Currently, Shannex communities have an accommodations-based relationship with residents who call our locations home. West Bedford Transitional Health will serve patients as they work with allied health professionals to get on a path back to home or the community.
Patients will be transferred from an acute care environment (e.g. hospital) after being referred directly to West Bedford Transitional Health by staff at Nova Scotia Health. Patients may also be directly admitted from a home environment. This is not a walk-in community.
Patients will stay with us from a range of days to weeks, and will depend on their personalized care plan. No matter how long a patient stays with us, their experience will be transformative and focused on a temporary treatment plan that enables them to return to a place they call home – this can include their own home or with loved ones, a community-style environment (e.g. retirement living community) or a long-term care environment.
