An Innovative New Model of Care
Shannex has partnered with Nova Scotia Health (NSH) to bridge the gap between hospital and community care. The first of its kind in Nova Scotia, West Bedford Transitional Health serves patients 18+ who no longer require a hospital stay, but need additional support before they return to community.
Adding this new service is reducing emergency wait times and capacity challenges faced by Nova Scotia’s health care system by freeing up hospital beds.
West Bedford Transitional Health
Since opening in Fall 2024, Shannex team members have helped to shape this innovative model of care in Nova Scotia while providing service to patients who otherwise would have remained in hospital. West Bedford Transitional Health will soon expand from 68 to 178 patient rooms—each designed for privacy and comfort with air conditioning, ergonomic furniture and a private washroom. Patients enjoy an environment that’s peaceful, beautiful and taken care of by our building and support services teams, ensuring everything they need is at their door so they can focus on returning home.

Individualized Assessment and Care Planning
Transitional health combines medical, rehabilitation, and practical and emotional support in a community setting. Patients work with their care team, including occupational therapists, physiotherapists, and other health professionals, to prepare for their return to home.

Nursing Staff
At West Bedford Transitional Health, Licensed Nursing staff include NPs, RNs and LPNs, and are integral supporting the goal of patients returning home. Together, they promote health, engage in inter-professional collaboration, perform regular assessments, and offer evidence based clinical expertise to inform care plans that meet patient/family goals.

Certified Continuing Care Assistants (CCAs), referred to as Transitional Care Assistants (TCAs) provide personal care with a focus on supporting and encouraging the level of functional ability required to return to community.

Restorative health professionals (e.g. PT/OT and PT/OT Assistants) support the focus on functional abilities, encouraging adherence to treatment plans, and guiding patients with their expertise and knowledge.
Meet the Transitional Health Leadership Team
In addition to clinical, care, and allied health professionals, patients enjoy peaceful and welcoming spaces, as well as a dining program built to fuel their recovery. Each member of the leadership team brings previous experience across a range of environments, including acute care, emergency care, long term care, retirement living and administrative environments. Together, and with patients at the heart of every decision, they are improving the Nova Scotia healthcare system by relieving pressure on hospitals and providing patients a personalized pathway to go home safely, confidently and with independence.
