Independent Living
For residents living in a Lifestyle Apartment or an All-Inclusive Lifestyle at either Parkland or Faubourg du Mascaret.

Assisted Living
For residents living in Assisted Living (also referred to as Special Care in New Brunswick) at either Parkland or Faubourg du Mascaret.

Memory Care / Enriched Care
For residents living in a Parkland Memory Care or Parkland Enriched Care Lifestyle Option.

Long Term Care
For residents living in a Shannex Enhanced Care nursing home in Nova Scotia or New Brunswick.
Family Member Survey
While any family member is welcome to complete the survey, this survey was designed specifically for family members of residents in a Supportive Living environment (nursing home, memory care or enriched care). Some questions may not apply to you or your loved one (please leave these blank).

Frequently Asked Questions
The Quality-of-Life Survey is an annual questionnaire that measures your overall resident experience at Shannex, Parkland Retirement Living or Faubourg du Mascaret communities. Our Quality of Life Survey was created by interRAI, a collaborative network of researchers and practitioners in over 35 countries committed to improving care. The team developed this survey tool in 2014, based on the best available evidence regarding aspects that directly influence seniors’ quality of life. The information we receive from the Quality of Life Survey allows us to make improvements in areas that matter most to residents.
You should only answer questions on the survey that you feel are applicable and appropriate to your situation and any question on the survey can be skipped.
These reviews help us identify opportunities to improve our services and your experience living with us.

We offer two types of surveys to gather valuable feedback: one for residents and one for family members and loved ones.
- The Resident Survey is tailored to the type of service a resident receives and may be divided into one of four service-line-specific versions. Residents may complete the survey on their own or with support from a loved one, team member, or volunteer, if the responses reflect the resident’s own feedback.
- The Family Survey is available for family members or loved ones who would like to share their own experiences and perspectives.
We encourage participation in both surveys to help us continue improving the care and services we provide.
If a resident cannot complete the survey independently, assistance can be provided by a team member or volunteer from any Shannex owned location. Please ensure only one resident survey is completed per resident. Family members or loved ones may also assist a resident in completing the survey, based on the resident’s personal experiences. If the resident cannot communicate their answers, the Family Survey is available for loved ones to complete based on your experience.
Once you have completed the survey, please submit it at the front desk and a member of the team will input your answers online. Your privacy is important. If you are not comfortable submitting your survey to a team member at your location, please contact brand@shannex.com and we would be happy to set up an appointment with you to complete the survey over the phone.

The resident survey takes about 20–25 minutes to complete, and the family survey usually takes 10–12 minutes.
